Replaces the COVID-specific item 63399 with a broader cardiac MRI indication for suspected myocarditis. Now eligible across 3 clinical contexts: acute heart failure, unexplained arrhythmia, and suspected drug-induced myocarditis, when echocardiogram, troponin, and chest x-ray are inconclusive.
